Sport Club Forms and Resources

There are multiple club resources in the table below. Please view the descriptions of the resources underneath the table for more information on each item.


Sport Club Form and Resource Descriptions

Accident Report If someone requires first aid or emergency personnel while participating in a Sport Club activity, this form should be filled out on site and submitted within 24 hours of the accident.
Additional Funding Request In the event there is additional funding available for clubs, all clubs, provisional or recognized, can apply for additional funding after exhausting the club's annual budget.
Advisor Contract A faculty/staff advisor is required for each sport club and must fill out this agreement. Your club will then return it to the Competitive Sports office during your Fall Individual Club Training meeting.
Coach/Instructor Contract  If your club recruits a coach/instructor, that individual must complete and submit this form (along with the Volunteer Agreement) before beginning to serve as a coach/instructor. 
Community Service Follow-up Form  Clubs that complete community service hours should submit this form within 5 business days of the service activity. 
Constitution Requirements Utilize this example constitution if you are trying to revamp or create a constitution for your club. Required language is noted in the example.
Donation/Dues Report  Submit this form if your Club receives an unsolicited gift from a donor. The gift could be monetary, club equipment or services rendered. You will also submit this form if you collect club dues.
End of Year Report  This report gives the Competitive Sports office information about how your club performed throughout the academic year and provides us with an update about with who will be your club’s new leadership team for the coming year. 
Event Request If your club is conducting an activity outside of regular practices/meetings, this form must be submitted 15 business days prior to the event. Any event outside of the Pensacola area (>25 miles) is considered travel, in which case you would need to fill out the Travel Request form instead. All events, on campus, off campus and virtual will be submitted through this form.
Event Participation Request If your club is attending an event that someone else is hosting on campus, off campus (within 25 miles of Pensacola) or virtually. This form should be filled out at least 5 business days prior to the event.
Event Follow-up This form will describe how your planned event went and must be submitted within 5 business days of the event. 
Fall Report This report gives the Competitive Sports office information on how your club performed throughout the Fall semester. Your club will also use this form to update the Competitive Sports office with who will be your club’s new leadership team for the coming Spring.
Fundraiser Event Request This form must be completed 15 business days before the fundraiser and approved before the club can act on a fundraiser idea.
Fundraiser/Solicitation Follow-Up This form must be completed within 5 business days of the event. 
Fundraising Guide Use this resource as a guide to help spark fundraising ideas for your Sport Club.
Incident Report Use this form if an incident happens during a club event, practice or meeting. An incident is anything out of the ordinary that alters the event from going as planned. Examples include but are not limited to equipment not working in recreation spaces, an altercation between participants, the sprinklers coming on during an event, etc.

Informed Consent- UWF Members

Any UWF student, faculty or staff member that would like to participate in any club event/practice, must register and join the Club’s roster at uwf.edu/imleagues before participating.
Informed Consent- Community Member Any community member that would like to participate in any club event/practice, must fill out this form with the club before participating. 
Informed Consent- Community Minor Any community minor that would like to participate in any club event/practice, must fill out this form with the club before participating.
Monthly Recap Submit this form by the 15th of every month to let the Competitive Sports office know how regular practices and meetings went for the previous month.
Officer Contact Information Submit this form at the beginning of the academic year and if there has been a change to your Club leadership during the semester.
Officer Contract Each student holding an officer position will need to submit this agreement signed at the Individual Club Training in the Fall semester or immediately throughout the year after Club leadership changes.
Practice Request Use this form to request Recreation space for club practices and to notify the office of when your off campus practices will be held.
Prospective Club Packet If you are interested in creating a new club, use this packet to help get you started. Contact the Competitive Sports office to get more information on the process of creating a club.
Purchase Request Complete this form in order to use SGA allocated funds on club purchases. Send the completed form to sportclubs@uwf.edu.
Social Media Post Request Recreation will post on our social media accounts for your club. Submit this request along with the picture(s) you want to be used in the post.
Solicitation Request If your club would like to solicite members of the community for money or goods, you will fill out this form. You cannot act on the solicitation until approved.
Sport Club Council Contract The elected Sport Club Council Representative will fill out this agreement and submit it during the Fall Club Officer Training.
SCC Executive Board Contract The elected Executive Board will fill out this agreement and submit it during the Fall Club Officer Training.
Sport Club Handbook Everything you need to know about running a sport club is in this resource. The handbook describes policies and procedures for sport clubs as well as lays out a schedule of required trainings and paperwork.
Travel Follow-Up Submit this form within 5 business days after you return from a Club trip. You must submit original detailed receipts with the form in order to be eligible for reimbursement. Make sure you've filled out the Travel Request for prior to your travel event.
Travel Quiz Each Club must designate a Main Contact for the trip when traveling outside of the Pensacola Area. That individual must complete the Travel Quiz each semester before being approved to travel.
Travel Request Submit this form at least 5 business days before your Club would like to travel outside of the Pensacola area (>25 miles). 15 business days before if SGA funds are being used. Each traveling participant must be a current club member and be on the club’s roster on IMLeagues.
Website Updates Use this form to request updates to your Club’s webpage on the UWF Recreation site.
Volunteer Agreement If your club recruits a coach/instructor, the Competitive Sports Office must submit this form for the Volunteer after the Coach/Instructor Contract.
