Dean's Certificate Process
A Dean’s Certification, sometimes called a Disciplinary Clearance or Letter of Good Standing, pertaining to UWF disciplinary records, can be completed when a student submits the proper forms to the Assistant Dean of Students for Student Rights and Responsibilities.
Receipt of these documents will allow Student Rights & Responsibilities to report UWF disciplinary record(s) to requesting entities such as institutions of higher education, professional schools or federal or state government agencies. In all cases, an authorization to release information is required.
A Dean’s Certification, Disciplinary Clearance or Letter of Good Standing is requested for a variety of reasons, including, but not limited to:
- Transferring from UWF to another institution
- Acceptance into an academic program
- Study Abroad
- Background information for new employees
- Background information for law enforcement
- Membership or to hold office in a student organization
- Scholarships
- Volunteering
- Application for the Bar
Dean’s Certification Process
Please abide by the following guidelines to submit.
- If your form requires graduation verification, transcript requests, GPA, eligibility to return, or any type of academic record verification, please send your form directly to the Registrar’s Office, and they will forward it to Student Rights and Responsibilities once it’s complete. Please contact them directly for information on the releases they require.
- If the requesting institution/agency only needs UWF disciplinary records and provides you with a form requesting your disciplinary records, please complete the top portion of the form and upload it to the release so we can process this request.
- If the institution or agency requiring a Dean Certification/Letter of Good Standing does not provide you with a form, please fill out the Disciplinary Information Request form. This will allow us to send a Letter of Good Standing.
Important
- We will not process requests without authorization.
- Our Procedures allow us to email these documents to colleges/universities and law enforcement only.
- UWF will not provide envelopes or stamps for documents that are to be mailed to the institution/agency. When providing an envelope, be sure to leave the return address blank, as our address will be the return address.
- Please be advised that all Dean’s Certification requests will be sent directly to the requesting agency upon completion.
- Student Rights and Responsibilities does not release prior conduct information over the phone, as we are unable to verify the identity of the caller. If you are unsure of your prior conduct history and would like to review it before Student Rights and Responsibilities releases it, please submit a Disciplinary Information Request form.
*All forms must be submitted ten to fifteen (10-15) business days prior to their deadline. For questions, contact the Assistant Dean of Student Rights and Responsibilities.
