Student Health Services Clinic Policies

Arriving Late/No-Shows / Cancellations/Rescheduling

Please arrive 15 minutes prior to your scheduled appointment time to ensure that you are checked in and have adequate time with a provider. If you are 5 minutes past your scheduled appointment time, you need to reschedule so that we may accommodate other students. Failure to show for a scheduled appointment will result in a $25 fee being charged to your student account. If you are unable to keep your appointment, please call and cancel or reschedule your appointment to avoid this charge. If you are unable to keep your appointment or no longer need to be seen, please call within 24 hours to cancel your appointment so that we can offer this time to another student. We do our best to see you in a timely manner, however, you may experience a short wait if unexpected situations arise. Please consider this when scheduling your appointment so it does not interfere with your class schedule.


Class Absence Appointment Slips

Students who miss class due to an illness and have a scheduled appointment at Student Health Services (SHS) on the same day are entitled to a note stating they were seen in the clinic. Students will have one week from their appointment time to obtain a note in case they forget to ask for one on the day they are seen. No notes will be given to students after that time.

Confidential Records Release Request

We are only able to provide your medical information to anyone, including your parents, with your written consent if you are age 18 or older.

In order to request a copy of your confidential records from Student Health Services (SHS), please complete the Authorization for Release of Confidential Information. In order to protect your privacy, the form must be notarized so we can verify we are releasing the information to the correct person. You can scan the notarized form and email it to SHS at healthservices@uwf.edu, fax it to 850.857.6100, or mail it to:

Immunization Records Form

An official stamp from a doctor’s office, clinic, or health department AND an authorized signature must appear here for any immunization records listed above to be valid.

Immunization Exceptions and Waivers

Fully online students who will not be taking any courses on a UWF campus or location are not required to provide proof of immunization but should submit a waiver through the MyUWF Immunization Status App. If the student enrolls in a face-to-face course or has to come on campus for any reason, the student will be required to submit the required immunization documentation.


Hepatitis B

Students wishing to decline this vaccine must read the CDC's Hepatitis B Vaccine Information Statement, select "Hepatitis B Waiver" and "Decline Vaccine" on the online immunization form indicating they have read the information and still wish to decline the vaccine, and electronically sign the form. Selecting "Decline Vaccine" and signing the form indicate the student understands the possible risk in not receiving the vaccine, including the potentially fatal nature of this disease. If the student is under the age of 18 and wishes to decline the Hepatitis B vaccine, a parent or guardian must provide their signature in addition to the student's signature in order for the waiver to be valid.


 Meningococcal Meningitis

Students wishing to decline this vaccine must read the CDC's Meningococcal Meningitis Vaccine Information Statement, select "Meningitis Waiver" and "Decline Vaccine" on the immunization form indicating they have read the information and still wish to decline the vaccine, and electronically sign the form. Selecting "Decline Vaccine" and signing the form indicate the student understands the possible risk of not receiving the vaccine, including the potentially fatal nature of this disease. If the student is under the age of 18 and wishes to decline the Meningococcal Meningitis vaccine, a parent or guardian must provide their signature in addition to the student's signature in order for the waiver to be valid.
