How to Make an Appointment
To make an appointment, please call our office at 850.474.2172, Option 1, during office hours (Monday - Friday, from 8 a.m.-5 p.m.) or in person at our office (Building 960, Suite 106). It is recommended that you call as early in the day as possible.
Please be advised parents and guardians cannot make appointments for their students.
Be Ready to Provide the Following:
- A reason for your visit - provide as much detail as possible (symptoms)
- A timeline of when you began to experience symptoms (when did symptoms begin)
- Have your schedule ready when making an appointment with our office.
Please make preparations to arrive 15 minutes early.
Things to Bring with You to Your Appointment:
- Your Student ID
- Insurance card (not necessary, but good to have available)
