Dental Insurance


UWF offers you comprehensive dental coverage through an array of dental plans. Each plan is designed to meet the needs of employees based on their individual plan usage, flexibility in using network or non-network dentists and cost.

Types of Plans and Covered Services

The dental plans are divided into four categories: PPO, Indemnity, Indemnity with PPO and Prepaid.

The following information will provide highlights of all the options, focusing on how the plans work. For additional information on each plan, consult the Dental Plan Comparison Information. The plan documents can be found on the MyBenefits Dental Insurance site and can provide additional information, to include monthly premiums, out-of-pocket costs and answers to specific questions you may have about coverage.

PPO/Indemnity Plans are the most secure and flexible of the available options. Indemnity plans offer a scheduled reimbursement amount (set fee) for covered services from any dentist or specialist.  PPO plans allow for discounts or greater benefit amounts on services with dentists in the network.


Eligibility

Dental insurance is available to all benefit-eligible employees (including eligible OPS employees). Employees pay the full premium amount on a pre-tax basis and have 60 days from their date of hire to enroll. Plan changes are only available during the annual open enrollment or as a result of an approved qualifying status change.

How to Enroll

  • Choose a dental company and plan.
  • Complete dental insurance enrollment with People First.
  • Submit a confirmation statement to the Human Resources Benefits Section to ensure proper setup of payroll deductions. You may send these by email to hr@uwf.edu or by Campus Mail to HR in Building 20E, Room 117.

Preventative PPO Dental
  • Flexibility to receive care from in- or out- of network dentists. (in-network=lower costs)
  • You generally have an annual deductible to meet before the plan starts paying benefits and an annual maximum benefit amount of $1,000 per person.
  • Basic services covered 50-80%.
  • Major services/orthodontia not covered.
  • Premiums range approximately $20-$65 per month.
  • Ameritas Preventive PPO (4023)
  • MetLife Preventive PPO (4033)
  • Humana Preventive (4094)
Standard PPO Dental
  • Flexibility to receive care from in- or out- of network dentists. (in-network=lower costs)
  • You generally have an annual deductible to meet before the plan starts paying benefits and an annual maximum benefit amount of $1,500 per person. 
  • Basic services covered  50-80%.
  • Major services covered 30-50%
  • Orthodontia: Covered for adults and children. (subject to maximums; waiting periods apply)
  • Premiums range approximately $30-$110 per month.
  • Ameritas Standard PPO (4022)
  • MetLife Standard PPO (4032)
  • Humana Standard PPO (4092)
Indemnity with PPO Dental
  • Flexibility to receive care from in- or out- of network dentists. (in-network=lower costs)
  • You generally have an annual deductible to meet before the plan starts paying benefits, and an annual maximum benefit amount of $2,000 per person. (Sun Life caps out-of-network at $1,500)
  • Basic services covered 80%.
  • Major services covered 50%.
  • Orthodontia: Child-only orthodontia covered by Sun Life
  • Others: covered for adults and children.
  • No orthodontia waiting periods.
  • Premiums range approximately $45-$145 per month.
  • Ameritas Indemnity w/ PPO (4021)
  • Sun Life Indemnity PPO (4074)
  • MetLife Indemnity w/ PPO (4031)
  • Humana Indemnity PPO (4090)
Indemnity Dental
  • Receive care from any dentist.
  • You have a deductible to meet and then seek reimbursement for part of the cost for the services you receive.
  • Humana Schedule B (4084)
Prepaid Dental
  • Pays benefits only when you use network providers; list is very limited, with no guarantee providers will accept new patients or have availability.
  • No deductible or annual maximum.
  • Most preventive care at no charge.
  • You pay a fixed copayment for dental procedures listed on the copayment schedule.
  • Orthodontia: Covered for adults and children.
