Forms
Potential New Members
- FSL Info Session - The virtual FSL Info Session must be completed in order to be eligible for fraternity or sorority membership. The steps to enroll in the Canvas course are on the FSL homepage.
- Hazing Prevention Module - Must complete the "Hazing Prevention: It's Everyone's Responsibility 101" module in order to be eligible for fraternity or sorority membership.
- Hazing Regulation Notice - Must sign in order to be eligible for fraternity or sorority membership.
- Recruitment/Intake Grade Release Form - The Recruitment/Intake Grade Release Form is used to verify and release potential new member (pnm) academic information for recruitment purposes to chapters. This must be submitted and approved prior to extending a bid or membership invitation. PNMs participating in a formal recruitment process do not need to complete this form, as it is included in the online registration.
New Members
- New Member Academic Release - Each fraternity or sorority new member must complete this form within 48 hours of becoming a new member. This form should be submitted by any prospective members along with the membership intake section of the FSL Action Sheet for all organizations conducting intake before the organization starts their process.
- New Member Institute - Each new member must attend the in-person New Member Institute (NMI) the semester they join (NPC/IFC) or the following semester (NPHC/MGC)
Active Members
- Online Hazing Prevention Module - Each current active fraternity or sorority member must complete the "Fraternity & Sorority Life Module" by the end of the third week of each fall semester.
Chapter Officers
- FSL Membership Intake Form - Chapter Presidents must submit this form to inform the Fraternity and Sorority Life office when your chapter plans on conducting intake. This form must be submitted by September 1 and January 15 each year.
- FSL Chapter Grade Report Request - Chapter Presidents can request a copy of their chapter's grade report each year. The Fraternity and Sorority Life office will not provide the chapter with their grade report unless it has been requested through this form.
- Event with Alcohol Report - Chapter Presidents or event organizers must submit this form within 24 hours of any events with alcohol.
- Roster Update Form - Chapter Presidents or Vice Presidents must complete this form by September 1, December 1, January 15, and May 1 each year.
- Officer Update Form - Chapter Presidents must complete this form by December 1 and May 1 each year.
- Advisor Update Form - Chapter Presidents must complete this form by August 1 and January 1 each year.
- End of Semester Report - Chapter Presidents must complete this form by December 15 and May 15 each year.
- Constitution, Bylaws, & Crisis Management Plans - Chapter Presidents must be completed by September 1 and February 1 each year. Any updates during the semester should also be submitted.
- FSL Funding Request - FSL has limited funding to support our chapters and councils who wish to provide programming which benefits the growth or development for the FSL community or for members who wish to attend an FSL conference.
- FSL Academic Plan Submission Form - Chapter Academic/Scholarship Chairs should complete this form by the second week of classes each semester if their chapter is on Academic Warning or Probation
