Credit Hours Used Form
If you were awarded the scholarship from FCLI, you must submit the correct portion of the Credit Hours Used (CHU) form to this office within 10 days after registration. Registration period is considered to include the drop/add period of your Florida institution. The CHU form should be submitted within 10 days of the end of drop/add for each semester. Failure to return the form will result in the forfeiture of your out-of-state waiver and disqualifying you from applying the waiver in the future. If you can not submit it directly online, please download the form, complete and mail it to the FCLI co-director responsible for your tuition exemption award. Students who register for less than the number of hours awarded must send a copy of the registrar's receipt, or other proof of enrollment, to the Florida-China Linkage Institute office, so that we may award the unused hours to another student. Failure to do so may result in cancellation of the exemption.
Note: The number of credit hours used must not exceed the number of credit hours awarded for a given term.
