Continuing Education Accommodations

Student Accessibility Resources (SAR) works with Continuing Education (CE) students who have disabilities to provide reasonable accommodations that support equal access to educational experiences.


Steps for Requesting Accommodations for CE

  1. Complete the CE Accommodation Request Form
  2. Provide supporting medical documentation.

Documentation Requirements

To be eligible for services, students must provide appropriate documentation from a licensed or certified professional who is qualified to diagnose and treat the student's disability. 

Please Note: IEPs and 504 Plans are typically not sufficient to establish the presence of a disability, but may be submitted as supplemental information.

Requirements
Professional Diagnosis
Documentation should clearly state the diagnosis, explain how the condition may impact the student in an academic setting and provide recommendations for reasonable accommodations.
Provider Information
Documentation should be on the provider's letterhead and include the provider's name, title, dates of treatment, signature, and professional license and/or certification number.
Qualified Professional
Documentation should be provided by a professional who is qualified to diagnose and treat the student's condition and has direct knowledge of the student's history, symptoms and functional limitations.
Age of Documentation
Documentation should generally be less than five years old. Exceptions may apply for certain permanent or stable physical disabilities, depending on the nature of the condition and the information provided.

 

Who can request CE accommodations?
Students enrolled in a UWF CE course or program who have a documented disability may request reasonable accommodations through SAR.

When should I request accommodations?
Submit your accommodation request as early as possible after registering for your course. Early requests provide sufficient time to review documentation, determine eligibility, and coordinate approved accommodations before your course begins.
