UKCOH Community and Alumni Awards criteria and requirements.
The Community and Alumni Awards celebrate and strengthen collaboration between the Usha Kundu, MD College of Health and the larger community UWF serves. Eligible nominees include alumni, businesses, organizations, agencies, and individuals, as well as UWF administrators, staff, faculty, and students.
2025 award nominations open Monday, October 28, 2024, and close Friday, January 17, 2025. The nomination committee will meet in January 2025.
Award winners will be recognized on Tuesday, March 11, 2025, at the Community and Alumni Awards Banquet hosted by the Usha Kundu, MD College of Health at the UWF Conference Center.

Alumni Awards
This award will recognize an alumnus who has demonstrated significant achievements and/or leadership in healthcare. This award celebrates individuals whose personal or professional accomplishments have contributed to advancing healthcare in meaningful and impactful ways.
Award Details and Criteria
- The recipient must be a University of West Florida alumnus from any college, not limited to the Usha Kundu, MD College of Health. This inclusivity ensures that UWF alumni from diverse academic backgrounds are eligible, provided their achievements and leadership relate to the healthcare sector.
- Leadership in healthcare organizations, public health initiatives, or health-related community service.
- Demonstrated commitment to healthcare education, policy reform, or advocacy that enhances the overall quality or accessibility of healthcare services.
- Any contribution that has positively impacted the healthcare field through clinical work, administration, public health, or healthcare entrepreneurship.
- Alumni may only receive this award once in their lifetime.
This award will recognize an individual who has made significant contributions to the field and/or displayed exceptional community service early in their healthcare career. This award highlights the promise and potential of emerging healthcare professionals who have shown exemplary leadership, innovation, and dedication to their field, even in the early stages of their careers.
Award Details and Criteria
- One letter of recommendation is required to support the nominee’s qualifications. This letter should clearly outline the nominee's specific contributions to community engagement in healthcare, detailing the scope and impact of their efforts.
- The recipient should have graduated from UWF Usha Kundu, MD College of Health/College of Health.
- Alumni may only receive this award once in their lifetime.
Community Awards
This award will recognize an external company, organization, agency, or individual who has significantly impacted the healthcare field through outstanding community engagement. The recipient's contribution should demonstrate a commitment to improving the community's well-being and fostering partnerships that enhance healthcare services, access, or education.

- The nominee's contributions should showcase a high level of innovation, leadership, and dedication to healthcare.
- The contributions must demonstrate tangible outcomes that have positively impacted the community, whether through increased access to healthcare, public health initiatives, healthcare education, or support services.
- A single letter of recommendation is required, serving as a testament to the nominee’s significant accomplishments and leadership within healthcare. This letter should highlight the nominee's specific contributions, achievements, and impact in their respective role, whether on a local, national, or global scale.
This award is designed to recognize an individual who has demonstrated outstanding leadership in the healthcare field. It should honor someone who has significantly contributed to healthcare through visionary leadership, innovation, and dedication to improving patient care, healthcare systems, or public health.
Award Details and Criteria
- The nominee must be a healthcare professional or individual who holds a leadership role in a healthcare organization, public health entity, or related field.
- The nominee should have a proven track record of inspiring, influencing, and leading teams or organizations to achieve excellence in healthcare services or innovation.
- The nominee’s leadership must demonstrate a commitment to improving healthcare outcomes, patient safety, or the quality of healthcare delivery.
- Contributions should reflect long-term impact, demonstrating sustainability and an ongoing commitment to the community and healthcare sector.
- One letter of recommendation is required to support the nominee’s qualifications. This letter should clearly outline the nominee's specific contributions to community engagement in healthcare, detailing the scope and impact of their efforts.
All are encouraged to submit nominations. Self-nominations are accepted. All awards require a letter of recommendation and may have certain criteria. Nominations leaving off required information may be disqualified. Letters of recommendation are not required to be from a UWF/UKCOH alum. Recommendations must be written by someone other than the nominator. The Selection Committee will review and select recipients for all Community and Alumni Awards. The committee reserves the right to change the category of the nomination based on the information submitted.
