- Items sent to the MPH Program Director:
- Letters of recommendation - Three (3) letters of recommendation are required.
- Letter of intent - This letter describes the student's
area of interest within public health, relevant past experience, future objectives, and the names of UWF
faculty with whom they have spoken regarding the MPH program.
- Personal writing samples - Two (2) personal writing samples are required
for each applicant (e.g., written reports completed by the applicant or other representative samples of professional writing skills).

- UWF Graduate Admission Application - This can be accessed online by clicking here.

On this application, students need to specify the Master of Public Health program.
- Official transcripts - Two sets from each institution attended.
- Graduate Record Exam - Scores within 5 years preceding application to the UWF MPH program.
- Official test results must be mailed directly from the Educational Testing Service to UWF's Office of Admissions.
- Applicants with terminal degrees (PhD or EdD) or advanced professional
degrees (MD, DDS, DVM, JD, DO) from accredited programs and licensed in the
United States may request to waive the GRE requirement.
