Each student accepted for admission must submit, prior to registration, a Medical History form and the required Immunization forms described below. These immunization and health form requirements were passed by the governing body of the Florida system and must be strictly adhered to by UWF.

Meningitis and Hepatitis B:
Effective July 1, 2008, all new UWF undergraduate and graduate students must provide documentation of vaccinations against meningococcal meningitis and hepatitis B or provide a signed waiver for each declined vaccination.
Measles, Mumps, and Rubella:
Students born after December 31, 1956 must submit documented proof of immunity to Measles (Rubeola) and German Measles (Rubella).
Submitting Your Health Information:
Submit your Medical History form and the required Immunization forms to the Student Health Center prior to your Orientation date so that you may register for classes at your Orientation Session.
You may submit the forms in person to the Student Health Center (Building 63) or to the Dean of Students Office (Building 21, Room 130). You may also submit the forms via mail, fax, or email (scanned attachment):

Failure to submit the required health and immunization documentation will result in a hold being placed on your student record which will prevent you from registering for classes.
