Immunizations

REQUIREMENTS

• All Students are required to show proof of having the following immunizations prior to registering for classes:
• 
  
  • MMR (Measles) 2 separate injections of MMR vaccine after the first birthday.
  • Cost $45.00
  • Hepatitis B - (3 separate injections) or waiver with signature
  • Cost $20.00 per injection
  • Meningitis - (1 injection) or waiver with signature
  • Cost $95.00
• All new students 18 or older who choose not to be vaccinated against either meningococcal Meningitis or Hepatitis B must sign a statement (waiver) that they have been made aware of the potential fatal nature of the diseases and choose not to be vaccinated.  All new matriculating minor students must provide signed parental consent to opt out of vaccination from either of these diseases.

Deliver all required documents to the UWF Student Health Center Bld. 63 or submit via email, fax or regualr mail.

Please allow 2 - 3 business days for immunizations to entered into the computer.

The following vaccines are also available in the Health Center:

• Flu (Influenz) vaccine - Offered annually during winter semester.
• Hepatitis A. vaccine - Cost $45
• TD-Tetanus Diptheria vaccine - Recommended every 10 years - Cost $35
• Chicken Pox - 2 doses, 12 months to 12 years of age. - Cost $75
