Graduate Readmission form (pdf):

Graduate students not in attendance during three or more academic semesters (including summer semester), but less than five years, must complete the Application for Readmission and provide any required documentation. The form must be filed according to readmission deadlines stated in the current Academic Calendar.

Graduate students who last attended their graduate programs over five years ago must reapply to their program.

Transcript Request form (pdf):

It is the responsibility of the applicant to submit two official transcripts from each college and university attended. Applicants may use the attached form to request their transcripts from the previous institutions they attended.

Graduate Major Track Change form (pdf):

Graduate students may change their track or specialization within the same degree program by completing the attached form. Graduate students desiring a new program must complete a new application for graduate admissions.

Use of Undergraduate Courses in a Masters Program (pdf):

A master’s program may include a maximum of six semester hours or two courses (whichever is greater in credit) of UWF undergraduate level course work. Graduate students must complete and submit this form to the Registrar's Office by the end of the drop/add period of the semester of enrollment.
