Use this checklist to keep track of your application.

      • Complete the Application for Admission to the University of West Florida http://uwf.edu/admissions  and submit the required documentation.   
      • Forward official transcripts for all colleges and universities and military medical training you have attended directly to the MLS Department Office Administrator (in addition to the UWF Office of Admissions).
      • Request 2 letters of recommendation using this reference form. One letter must be from a college or MLT instructor, and one must be from your current hospital laboratory director. References may be mailed to the address above, or emailed from the evaluator to vdubose@uwf.edu.
      • Complete the MLT to MLS Clinical Year Application-2018 and email to vdubose@uwf.edu.
      • Attend Skype interview with selection committee.

SELECTION CRITERIA

Applicants must be admitted to UWF. Students in the MLT to MLS degree must also meet the following requirements at the time of application:

        • EITHER Associate of Science in Medical Laboratory Technology degree from a community college AND completion of the prerequisites of the program and fulfillment of the UWF general studies courses
        • OR Military training as a Medical Laboratory Technician and an Associate of Arts degree to include the prerequisites of the program and UWF general studies courses
        • Minimum overall GPA of 2.5 on 4.0 scale
        • Certification as an MLT by ASCP or AMT
        • Submission of a departmental application by the published deadline  
        • Recommendation letters as requested
        • Skype interview with selection committee. The selection committee is made up of representative members from UWF faculty and education coordinators from participating hospitals.
